This Registration
Form must be filled out completely.

2. PARTICIPANT
REGISTRATION
You must register
with the Newfoundland Award Office prior to commencing any work toward your
Award. There is a registration fee of
$20.00 payable by all new participants and a $5.00 Record Book fee.
Upon receipt of the
$25.00, the participant will receive a record book, to keep for all three
levels of the award, and a receipt.
